Care Coordinator

Responsibilities

As a Care Coordinator, your primary role will be to support the smooth operation of the clinic and ensure an excellent experience for every patient. Here’s what you’ll do:

  1. Customer Service:
    • Answer phone calls, greet patients and visitors, and provide friendly and professional assistance.
    • Address patient concerns and questions with empathy and accuracy.
  2. Patient Intake and Records Management:
    • Collect detailed and accurate information during a patient’s first visit and throughout their treatment journey.
    • Assist patients in completing necessary paperwork, ensuring all information is correct.
    • Maintain and update medical records with accuracy and confidentiality.
  3. Appointment Scheduling:
    • Coordinate patient appointments, ensuring smooth scheduling and time management.
    • Track coverage approvals for all funding streams to support patient care plans.
  4. Administrative Duties:
    • Manage incoming and outgoing communications, including emails, mail, and faxes.
    • Process payments at the time of service and handle invoicing tasks.
    • Organize courier packages and provide administrative assistance to the Care Coordinator Manager and Clinic Director.
  5. Data Entry and Billing:
    • Accurately enter data into the clinic’s systems.
    • Ensure timely billing for services provided.

Qualifications

To excel in this role, you should bring a mix of skills, experience, and personal qualities.

  1. Educational Requirements:
    • High School Diploma, G.E.D., or equivalent qualification is required.
  2. Essential Skills and Experience:
    • Excellent customer service and telephone etiquette.
    • Strong organizational skills with the ability to multitask effectively.
    • Proficient in data entry and basic computer applications.
    • Clear and professional communication skills, both written and verbal.
  3. Personal Attributes:
    • Flexible and adaptable to changing priorities.
    • Friendly, approachable, and patient-focused.
  4. Preferred Experience:
    • Previous experience in a physiotherapy clinic or similar healthcare environment is an asset.
