2. Responsibilities
As the Patient Care Coordinator, your day-to-day duties will include:
- Managing patient flow: Plan and coordinate patient intake, discharge, and referrals to ensure a smooth patient journey.
- Supervising staff: Oversee nursing and support staff, ensuring high-quality care and efficient operations.
- Scheduling staff and managing workloads: Assign tasks, adjust schedules, approve leave requests, and monitor overtime to meet staffing needs.
- Supporting patient care excellence: Promote team collaboration, guide staff through patient education efforts, and act as a resource for clinical best practices.
- Leading care conferences and rounds: Facilitate discussions with the care team to develop and optimize patient care plans.
- Mentoring and coaching staff: Provide support to nursing teams through skill assessments, feedback, and performance coaching.
- Handling patient concerns: Address patient and family inquiries or complaints, collaborating with the Quality Improvement team when necessary.
- Participating in risk management: Assist with safety audits, policy updates, and other quality improvement efforts.
- Collaborating on hiring decisions: Offer input on new staff selection and provide feedback on staff performance to the Manager.
- Monitoring operations: Evaluate patient care standards and ensure adherence to nursing guidelines.
- Participating in projects and research: Promote staff involvement in special initiatives and participate in research as appropriate.
3. Qualifications
- Education:
- Graduation from an accredited School of Nursing.
- Active RN registration with the British Columbia College of Nurses and Midwives (BCCNM).
- Post-graduate coursework or certifications related to the clinical area are an asset.
- Experience:
- Minimum three (3) years of clinical experience in a related field, with at least one (1) year of supervisory experience.
- Skills & Abilities:
- Strong leadership and problem-solving skills.
- Ability to coordinate patient care and manage daily operations effectively.
- Excellent communication and collaboration abilities.
- Proficiency in handling conflict situations with empathy and professionalism.
- Ability to manage staff schedules and ensure appropriate staffing levels.
- Familiarity with clinical software and equipment.
- Physical ability to perform job duties, including patient assistance when necessary.
